  5. Description of the problem—must be specific and must include the organization/healthcare entity problem
  6. Description of the intervention—must be specific and must include where or which organization/healthcare entity this intervention will be implemented
  7. Outcome measure section
  8. Process measures section (note: you need three in this section)
  9. Balance measure section
  10. Impact measure section
  11. Communication plan—answer all four questions and be specific.
  12. If you have any questions, ask your faculty.
