The assessor will use the criteria outlined in the following tasks to determine if you have satisfactorily completed this assessment event. Follow these instructions to ensure you demonstrate the required knowledge and skills.
Read all instructions carefully and complete all requirements of the assessment.
Part 1: Prepare and complete client file reviews
Preparation for assessment
Case Assessment
Conduct a thorough case assessment of the requirements of 2 people with co-existing needs, based on their review of the client files provided. You will be provided with templates to record your assessment which includes prompts and questions for you to respond to.
To do this you will need to:
- Gather existing information about each person by reviewing:
- Ron Jones client file
- Holly Brown client file
- Seek additional information to ascertain the range of issues the person is experiencing by consulting with a colleague. For this activity the assessor will arrange 2 students to work as colleagues to discuss the file review, to ascertain information you may have not identified. Refer to the class Teams site for further information.
- Using a variety of sources, identify and research the issues this person is facing.
From your review of each persons client file, your research and your discussions with colleagues and specialists, you are to outline:
- a minimum of 2 co-existing needs the person is experiencing, and an analysis of the interrelationship between these needs
- how these needs impact the person
- issues the person is facing
- strengths the person has shown.
Based on your assessment and analysis of the persons co-existing needs, using a strength-based approach:
- identify strategies appropriate for the person to consider
- gather appropriate information about services relevant to the person that you will provide to them during their assessment meeting
- identify appropriate referrals you will discuss with your assessor
- detail how the persons information is to be stored and ensure confidentiality is maintained.

Ensure you identify any tools you used, advice sought from specialists and colleagues, how your process aligned with the organisations policies and procedures, how you gathered existing information about the person and how you ensured this information was stored according to policy. (Your answer for each person should be between 150 – 250 words) |

b.Based on the information you accessed through your case file review, in particular your review of the persons client file, and information you could obtain from discussions with specialists and colleagues, what needs have you identified that the person has? |

c.In 350 to 500 words per person, provide an assessment of these needs and the impact of the interrelating issues these have on the person. |

d.Based on your evaluation and assessment above, research 5 services that you consider will be appropriate to provide to the person during your assessment meeting. Outline at least one (1) benefit of each service and at least one (1) reason that youve selected them and how you believe they are suitable for the persons needs. Clearly identify if there are any risk factors associated with the delivery of these services. |

e.Provide evidence of how you would organise and consult with the person being assessed. What assessment tools, forms, service information, policies and procedures would you access? What would you do to arrange the meeting and obtain consent? What information would you provide to the person regarding the assessment process? How would you ensure you provided an appropriate environment for the assessment meeting to occur? Monitor the outcomes for meeting the persons needs? (Your answer should be between 250 – 400 words) |
Part 2: Reflection
In consultation with a peer, you will:
- Outline the steps you have documented in the template provided in Part 1
- Discuss how you would prepare for the meeting for both clients
- Review the completed template provided in Part 2 during the meeting
- Discuss each individual plan and provide feedback
- Make notes in the template provided below of the outcome of the meeting.
Submit the completed template below to the teacher/assessor after the reflection meeting with any changes to the assessment based on the feedback you received and documented.
You are required to complete a referral for each of the clients, based on the analysis of needs identified for each client. Referral to other Service (out) templates are located in the Indigo Community Services and Health Hub
